The inquest continues into the death of Mr David Gray, who died following a massive overdose of diamorphine administered by an overseas doctor during an out-of-hours visit. Inquest Update The inquest continues at the Coroner’s Court in Wisbech, Cambridgeshire and a summary of evidence given so far can be found at www.anthonycollins.com. Evidence will be given by the following witnesses this week (Monday 25th January, Thursday 28th January and Friday 29th January). ‘Call’ refers to witnesses who are expected to give evidence in person and ‘read’ refers to witnesses who have provided a written statement which will be read to the court. Witnesses – Monday 25th January 2010 Dr Browning (call) - a General Practitioner in Suffolk, he also works as Clinical Governance Lead for Suffolk Doctors on Call/Take Care Now (TCN). Dr Browning will be giving evidence about the call he received on 17th February 2008 informing him that a patient had died after being seen by one of their doctors and receiving an excessive dose of a drug. He carried out an investigation and decided that TCN could not safely allow Dr Ubani to continue working for them. Dr Browning will also be asked to give evidence on the procedures that were in place at TCN in relation to the safe management of controlled drugs. Helen Palmer (call) – has been employed by Suffolk Doctors as their Drug Co-ordination Manger since November 2004. She will be asked to give evidence on her responsibility for maintaining the Controlled Drug Registers within the company and she will also be asked to comment on the Palliative Care Green Boxes supplied to Suffolk Doctors/Take Care Now by Heath Road Hospital Pharmacy in Ipswich. Shaun Moggan (call) - an Investigation Manager employed by the General Medical Council (GMC), his job is to consider complaints lodged with the GMC and to consider whether the nature of matters complained about are such that a referral should be made to the GMC’s Interim Orders Panel in order to protect patients and members of the public. Mr Moggan will be asked to give evidence about the complaint lodged in relation to Dr Daniel Ubani. Dr Ubani was present at the Interim Orders Panel on 8th August 2008 when his registration was suspended. Mr Moggan will be asked to comment on this. Dr Kennedy (call) - is a General Practitioner in Hayes in Middlesex and the Executive Medical Director of Take Care Now. He will be asked to give evidence on the screening process of doctors working for TCN’s Out of Hours Services. He will also be asked to give evidence on the induction process for TCN clinicians and in particular to comment on Dr Ubani’s induction. Dr Kennedy will be asked to give evidence about Palliative Care Boxes and controlled drugs carried in TCN cars. Thomas Stewart (read) - is a Company Director of Cimarron Ltd. Cimarron’s primary function is to supply general practitioners to primary care trusts. He will be asked to comment on the statement he provided to Cambridgeshire Constabulary on 26th February 2008 in relation to Dr Ubani who was being investigated regarding a patient’s death. Witness - Thursday 28th January & Friday 29th January 2010 Dr Uzokwe (call) - currently works at Barrack Lane Medical Centre, Ipswich. He will be asked to give evidence on Dr Ubani’s induction. This was Dr Uzowke’s first ever induction and the Coroner will want to determine whether he inducted Dr Ubani appropriately.
