“As clinical specialists in the care of older people, we welcome the publication of ‘Shaping the Future of Care Together’ and its focus on the details of how care should be delivered and funded. It is essential that we tackle the boundaries between health and social care and develop a holistic caring service which takes account of the social and medical needs of older people. An integrated approach is vital if we are to prevent unnecessary hospital admissions and the distress these cause to individuals and their families.
