We're honored to have been selected to provide technical support and training to community health centers across the country. This network cares for over 20 million Americans and already provides some of the highest quality care in the country. We hope to cultivate these additional quality tools to support their work in treating underserved and special populations.
This HRSA National Cooperative Agreement, the only one awarded nationally for workforce development, is an enormous honor for CHC. To transform primary care, we need an expert workforce trained to a high performance model of primary care. The award recognizes CHC’s expertise in health professions training and team-based care. It also builds on our experience through our Weitzman Institute in engaging organizations in a process of transformation and continuous improvement. We are excited about the ability to provide technical assistance and guidance to the more than 1,000 health centers across the United States.
In 2014, Dr. Bodenheimer, addressed the audience at our annual Weitzman Symposium on the topic of the shortage of primary care providers. During his visit to our health center he witnessed our model of care first hand. We are honored to be highlighted in this report as an innovator in healthcare delivery.
As part of their responsibilities, our RNs enroll patients with complex care needs in care management. A tailored nursing care management plan details problems, self-management goals, clinical targets, progress toward goals, and interprofessional collaboration in the care of that patient, particularly with behavioral health.
CHC is at the forefront of transforming primary care, and creating a new role for RNs in primary care. Their key focus areas are prevention, patient education, chronic illness care, and team support. We are incredibly proud of our RNs and committed to sharing our model with others.
CHEFA is proud to provide support for this program that uses innovative technology to provide critical increased access to treatment and better outcomes for children throughout Connecticut.
We are thrilled to receive support from CHEFA to help expand access to high quality behavioral health care for our children. School based health centers are a critical source of care for CHC’s pediatric patients. Project ECHO is an innovative new telehealth tool we’ve been pioneering at CHC, and we are excited to extend it to support our school based health centers.
CHC is proud to be the largest provider of school-based health services in the state for the last 20 years. These centers are a critical part of our work, because they allow us to reach more children and adolescents and ensure access to the highest quality of care during their formative years. We are grateful for our partners - UCONN School of Medicine Child and Adolescent Division, who is providing the expert staff; Connecticut Association of School-Based Health Centers, and the CT Chapter of the American Academy of Pediatrics – who will play a major role in the success of this project.

For 40 years, Community Health Center, Inc. has been providing primary health care to families across Connecticut. Today, we care for more than 130,000 patients, many of whom are children. Our studies show that programs – like Recess Rocks - that empower people are highly effective. They provide a forum for kids and their families to discuss their needs and define action strategies to address childhood obesity.
