The U.S. Consumer Product Safety Commission (CPSC) and five retailers are announcing a recall for crib tents and play yard tents made by Tots in Mind, Inc. due to strangulation and entrapment hazards, which may lead to injury or death.

Tots in Mind recalled the play yard tents in July 2010 and offered a repair kit that is no longer available. The company is no longer in business and no longer sells any products.

The CPSC is aware of 27 tent failures - including one death and one serious brain injury - that occurred between January 27, 1997 and April 2012.

The products were sold at Wal-Mart, Toys R Us, Babies R Us, Burlington Coat Factory, Amazon.com, and Bed Bath & Beyond.
